At North East Ohio Health Services we understand that assigning older individuals to the most appropriate treatment setting successfully prepares them to maintain maximum independence and quality of life. Utilization of the services provided in the Geriatric Program can enhance the therapeutic and cost effectiveness of medical/ psychiatric treatment efforts and can establish a viable productive partnership between the elder care system and the behavioral health treatment community. The program provides a sense of community within a safe environment for geriatric clients where each client is viewed as an individual with unique needs requiring individualized care planning. Varied needs among older clients necessitate access to and the involvement of professionally qualified interdisciplinary team members. At NEOHS this team includes psychiatrists, social workers, nurses, case managers, psychologists and other disciplines as needed.

The goals of this program are to identify and assist the older population with the services and care of a multidisciplinary team. Program staff are also available to speak to your group about the mental and emotional health aspects of aging and of caring for an older adult. |
