Goals
  • To ensure ease of access for all appropriate health consumers.
  • To provide a timely appointment for each client that meets admission criteria.
  • To provide an alternative referral if the client does meet admission criteria.
  • To ensure cases are assigned to an appropriate case manager, counselor, and/or physician in a timely manner.
Description
The Diagnostic Assessment begins with the initial call into the Agency by the client or their family member. A brief screening process is completed at this time and documented. The appointment for the diagnostic assessment is completed at this time.

A Diagnostic Assessment is completed on all new referrals and clients returning after long absences. The comprehensive assessment includes a medical/physical screen, a psychosocial history, presenting problems, treatment history and financial assessment.

Goals
  • To provide comprehensive medical psychiatric management and treatment.
  • To work collaboratively with the team of professionals involved with each client; i.e., case managers, counselors, psychologists, and/or nurses from within the agency and any that may be involved from other service areas/agencies.
  • To provide medication education, monitoring and informed consent to clients and as appropriate, to family members.
Description
The Med-Somatic Services are done by Board Eligible/Certified Psychiatrists and supervised Residents through the Public Academic Liaison (PAL) Program of University Hospitals of Cleveland and the Cuyahoga County Community Mental Health Board. The supervision of all physicians is completed by the Medical Director/Executive Director.

Goals
The provision of and coordination of evaluation, rehabilitation, and supportive services which leads to a reduction in symptoms of psychiatric illness, improves the client's access to community resources, and supports the client's return to an optimal level of functioning.
Description
CPST (Community Psychiatric Supportive Treatment) Services include a wide range of interventions with clients, their families and caregivers and other service systems, designed to promote the improvement and maintenance of optimal personal, social and community functioning of persons with severe and persistent mental illness (SMD adults and SED children and adolescents).

The CPST (Community Psychiatric Supportive Treatment) services are provided by a team of case managers, working collaboratively with their clients to develop comprehensive Individual Service Plans (ISP's) that promote the achievement of the client's personal goals in such major life areas as health and mental health, education and employment, income and housing, and family and social functioning. The case manager's role included planning (both with and on behalf of the client), direct services to the client (education, supportive counseling, symptom monitoring, etc.), and advocacy, referral and linkage with other community systems (health services, Social Security, supported housing, etc.).

CPST Services are targeted primarily to adult clients with severe and persistent mental disorders, but may also be provided, when medically warranted, to other clients of the Agency who would benefit from this intensive level of community support.
 

 

 

 
Goals
To provide brief-term, time limited counseling, psychotherapy and psychiatric services that are designed to meet the mental health needs of the client.
Description
The Individual and Family Services Program is designed to address a wide range of psychiatric and emotional issues. Program staff includes psychiatrists, licensed clinical social workers, counselors, and psychologists who are experienced in assisting the clients in dealing with depression, anxiety, stress-related disorders, and relationship issues, as well as psychiatric problems which are further complicated by substance abuse. Psychological testing by licensed clinical psychologists for clients with learning, attention, and psychiatric/behavioral disorders as well as assessment of adaptive behavior skills. Full interpretation of testing results with the client, family and other professionals is provided.

Population Served
Individuals residing in the community who are seeking counseling for mental health concerns. Counseling services are available for all ages. Services are provided in both individual and group settings and are based on client need. Clients may be self-referred, referred by a community provider, their insurance company, their physician, or their family member.

  • To provide especially focused and organized care with appropriate intensity to the more fragile and treatment-resistant SMD clients.
  • To provide the clients with access to those community resources which will improve their quality of life.
  • To provide the necessary support, education and treatment manage- ment to clients and their families to ensure treatment compliance and independent functioning.
  • To monitor the outcomes of the treatment process.
Description
The Comprehensive Schizophrenia Program philosophy is based on a holistic approach and utilizes education, medical management, and support to deal with the emotional, physical, and spiritual aspects of each client.

Goals
To provide a permanent residence for long term consumers of mental health services to maximize their individual and group level of independence and functioning, as well as their ability to integrate into the community.
Description
The Rose Home Residential Support Program was established as a cooperative venture of Connections and the Cuyahoga County Community Mental Health Board. The program is staffed twenty-four hours a day, seven days a week and is operated by Connections, which provides a full range of residential support services to the residents, including room and board, services for personal living, home needs and socialization skills; assistance with daily living and decision making skills; symptom monitoring for psychiatric illness, assistance in accessing needed health and medical services; and supportive contacts with residents' families or significant others. Connections is also the primary provider of community mental health services for the residents, through the Agency's main offices in Beachwood, including Medical-Somatic, CSP, and Diagnostic Assessment services. Those services are described elsewhere in the Agency Service Plan.

The Rose Home is located at 2435 West St. James Parkway, in Cleveland Heights, Ohio, 44106. The facility is located in a residential neighborhood, a short walk from a small retail/business district (known as the Cedar-Fairmount area in Cleveland Heights).
Population Served
The program and facility is designed for a capacity of twelve residents. The primary target population includes older adults (age 55 and above), diagnosed with serious and persistent mental disorders, with priority given to those who have had a long term admissions in the state hospital system, and who have been identified as having potential for successful transition to a supported community living situation.
 

 

 

 
Goals
  • Provide mental health treatment to children and adolescents. Treatment may include the following services: diagnostic assessment, psychological testing, counseling, case management, and medication.
  • Provide treatment that is accessible to clients in the community via home- and school-based treatment.
  • Provide group treatment to clients to improve emotional/behavioral functioning. Groups will be formed for specific purposes and issues.
  • Assure quality service by maintaining an active and ongoing review of program contents by the administrative program director.
Description
  • Counseling: Individual, Family, and Group counseling. This service consists of licensed counselors/social workers working with children, adolescents, and families to meet their needs and goals.
  • Case Management: This service includes assisting families with interpersonal difficulties, school issues, and linking families to services in the community. Outreach is provided where a case manager visits the home, school, and other community sites. A comprehensive psychosocial assessment is completed in addition to treatment.
  • Medical Somatic: This service offers the client ongoing evaluation and medication treatment by psychiatrists. Nurses also provide ongoing health monitoring and education.
  • Diagnostic Assessment: This service offers a comprehensive evaluation to determine which services would best serve the client. This assessment includes mental status, medical history, social history, family history and other information upon intake to the agency.
  • Psychological Testing: This service is provided by licensed clinical psychologists for preschoolers and children with learning, attention, and psychiatric/behavioral disorders. We can also assess adaptive behavior skills, and offer parent and school behavior surveys. Full interpretation of results with family and other professionals is provided.

Goals
The provision of outpatient alcohol, drug, and dual diagnosis services designed to assist clients in identifying and coming to acceptance of their addiction, initiating a process of recovery, effectively developing and utilizing personal, community and social support systems, and in promoting abstinence from alcohol and other drugs of abuse.
Description
  • Comprehensive assessment: to evaluate the nature and extent of the client's abuse or addiction to alcohol or other drugs, and to recommend appropriate treatment alternatives.
  • Med/Somatic: includes assessment and treatment of emotional or psychiatric disorders, assessing withdrawal potential, prescribing and monitoring medications.
  • Case Management: includes evaluation of client's community living skills, assistance with accessing relevant social, financial, and community supports to promote abstinence from alcohol and drugs and improved quality of life.
  • Individual and Group Counseling: structured, time-limited series of face to face sessions focused on attainment of treatment goals, and including both psychoeducational and cognitive-behavioral approaches to establishing and maintaining sobriety and recovery.
  • Intensive Outpatient Program (IOP): structured program of assessment, education, counseling and support, meeting 3-4 days/wk, 3 hrs/day, for clients needing an intensive level of support in establishing early recovery. This program specifically addresses the needs of deaf and hearing impaired clients who struggle with substance abuse disorders.
Location
Outpatient treatment services are offered at our office in Beachwood. Case management services and support may also be offered in the client's home and other community settings throughout Cuyahoga County.
Population Served
Chemical Dependency services in Beachwood are available to adults who meet the diagnostic criteria for Substance Dependence or Abuse as outlined in the DSM-IV. Connections has a particular mission to provide these services to adults with co-existing severe and persistent mental illness. The agency also provides specialized services to deaf and hard of hearing adults experiencing these substance use disorders.

To provide high quality mental health care to both pregnant and postpartum women who struggle with depression, anxiety, substance use, including nicotine, and other mental health issues.

Addresses issues and concerns such as; mood management, use of medication during pregnancy, substance use and smoking cessastion, changing relationships, parenting, and self care and seeking support.

  • Reduce the risk of future drinking and driving incidences through modifying the behavior and attitudes of program participants
  • To help participants understand how alcohol and other drugs impair driving performance
  • Learn how the use of alcohol and other drugs affect the individual, family, and community
  • To assess ones own personal patterns of alcohol and other drug use
Description
The Driver’s Alternative Program is a residential 72-hour alternative to jail for OVI offenders. The Program includes educational sessions, small group discussion, and assessment. The program is certified by the Ohio Department of Alcohol and Drug Addiction Services.
Location
