The Terms of Service is an agreement between you and North East Ohio Health Services (“NEOHS”). Your continued use of this website indicates your understanding and consent to the Terms of Service. If you do not agree with the Terms of Service you may not access or use this website.

  1. The contents of this website are copyrighted and may not be used or reproduced in any manner without the written permission of NEOHS. NEOHS assumes no liability or responsibility for any errors or omissions in the content of this website. Use of this website and the information on this website are at your own risk.

  3. Software made available for download from this website is subject to United States Export Controls.     • Employment Application: NEOHS collects the personal information you provide when you apply for a position through the ‘jobs’ page of our website. Submitting information via the ‘jobs’ page is purely voluntary and is not a requirement for employment. NEOHS uses the information you provide to determine your eligibility for the position that you have applied for. All information submitted will be kept confidential. This information will not be disclosed to any third parties except as necessary for background and employment checks.
  • How to change or remove your personal information
    • Verifying or Changing Your Information: If you would like to verify or change the personal information that you have submitted through www.connectionscleveland.org please send an email to webmaster@connectionscleveland.org.
    • Removing Your Information: If you have submitted personally identifiable information through www.connectionscleveland.org and would like that information deleted from our records, please send an e-mail to webmaster@connectionscleveland.org and we will delete the information from our electronic files and databases, and destroy all hard copies. The only exceptions are when we must retain your information to comply with local, State, or Federal laws, or when required to do so by a Court of Law.

Client’s Rights and Grievance Policy

The purpose of this policy is to protect and enhance the rights of persons applying for or receiving mental health services and or chemical dependency services by establishing specific rights of clients and procedures for responsive and impartial resolution of client grievances and any complaints about alleged violations of their rights relating to Protected Health Information.
